Welcome to the Joslin Diabetes Center Affiliate at Dubai Health Authority. We look forward to working with you to give your patients the care they need to prevent and manage their diabetes. Because we partner with the Department of Health, Government of Dubai, all Primary Care Physicians from PHC can refer their patients to us.
Primary care physicians and other healthcare providers refer their patients to us because they know we offer the best care for a disease that can be complex and time-consuming to manage. They appreciate our JoslinCareTM model and the range of specialty services we provide. As part of your patient's total healthcare plan, JoslinCareTM ensures that each patient receives personalized and individualized care based on his or her specific educational and medical needs.
Depending on the JoslinCareTM pathway that is developed for your patient, he or she may see a Joslin physician along with other members of our team for occasional or regular diabetes care and follow-up. Team members include our nurse educators, dietitians, exercise physiologists, podiatrists and others. In all cases, you will receive regular reports on treatment plans and visits. We recommend that all patients receive a full team consultation when referred to us. The team members include a board-certified endocrinologist, a diabetes nurse and dietitian educators, an exercise specialist and a podiatrist. However, if your patient is well-managed and you simply want access to diabetes education classes or to a dietitian for an individualized meal plan, we would be happy to accommodate those requests.
At present, the Joslin Diabetes Center Affiliate at Dubai Health Authority endocrinologists are caring for patients with type 1 and type 2 diabetes age 18 and older. A pediatric division is planned for the future. We would like to recommend that the following patients in particular be considered for a referral to Joslin Diabetes Center Affiliate Dubai:
 | All adults who are newly diagnosed, |
| | Patients with poor diabetes control, |
| | All patients with an A1C greater than 8 and strongly consider those with an A1C greater than 7, |
| | Recent or severe hypo- or hyperglycemia, |
| | Patients with additional risk factors such as cardiovascular disease, hyperlipidemia and hypertension, |
| | Patients who you think will need more intensive management, such as an insulin pump or who you think will need more thorough evaluation with continuous glucose monitoring for wide glycemic fluctuations. |
